ATHLETIC CLEARANCE
Athletic Clearance applications are now being accepted for the 2024-2025 Academic Year.
Parents/Students must use the Home Campus website to complete the application.
To complete the application process a 24-25 EGUSD Sports Physical will need to be printed, filled out and signed by the parent/guardian as well as completed and signed by the medical professional (MD, DO, PA, NP) after a sports physical has been scheduled.
If you have an existing account, Log In and select 24-25 School Year and update, otherwise use these to help you with the online process OR follow the steps below…
Click the link
- Click “For Students & Parents” box at top of page
- Choose California
- Either “Create an Account” or Login to previously created one
- Choose “Start Clearance Here” (blue box, upper right corner)
- Type Toby Johnson MS
- Select Year 2024-25
- Select or Add sports, next
- Complete all fields and submit application
MUST be completed/signed after May 1, 2024.
A new online Athletic Clearance is needed every academic year.
